My ALL time favorite would be meaningless as a "picture" without the context....
At a mexican restaurant in Houston, the door to the men's room is painted blue, and the door to the ladies' room is painted pink. yeah, rather sexist, I admit.
HOWEVER, once you are inside the men's room, and finish your business, and head toward the exit door of the restroom... the INSIDE of the men's restroom door is painted PINK and has a "LADIES" sign on it!
It has caught me off guard more than once! I have to look around quickly to see that there ARE, in fact, urinals!

I haven't been to a Max&Irma's restaurant in a while, but they used have on the men's room door a silhouette of a lady pointing to the lady’s room, and on the lady’s room door a silhouette of a man pointing to the men’s room.
