I wish to formally and officially deny any potential rumors of some NSA boot camp trainees getting drunk on acorn ale, sneaking into a secure area and accidentally firing one of the rail guns stored there. It is all fake news. As they said the hole originated from the inside eliminating any and all possibility of roguetech labs and bait shop from being legally responsible or culpable.

I'm always still in trouble again

Fake news. There couldn't be a hole drilled in the ISS 'cause the ISS is a hoax 'cause Man has never went into space.

With all of the telemetry on the astronauts and cameras everywhere it should be easy to figure out who did this. It isn't like there are a lot of people up there.

I believe they've found the culprit:

I love that initial fix was just a bit of duct tape.

The hole is in the Soyuz apparently. Marks indicate several attempts.

If it's not supposed to move and it does - duct tape.
If it's supposed to move and it doesn't - WD40

                        A serious response, finally!

Yeah, while the Soyuz is docked, it's considered part and parcel of "the Space Station" (first sentence below), but the hole does, indeed, appear to be in the Soyuz capsule (second sentence below), not the Space Station that stays in orbit.
