G'kar's "eye offended" Cartagia. Cartagia had it plucked out. Dr. Franklin provided him with a functional artificial replacement. Earlier, in a flash-forward to a point 15 or more years later in "War Without End," we saw G'kar with an eye-patch, when he and Londo killed each other.

Is there a canonical or JMS-official explanation for what happened to the eye-patch in those intervening years?

Supposedly the answer is in the Centauri Trilogy books. Haven't read them, but my guess would be that the artificial eye was lost/damaged at some point. He removes it a couple times after Dr Franklin gives it to him, so no reason for it to be permanently attached.
