Here's a polyptych. The Adoration of the Mystic Lamb by Hubert and Jan van Eyck which was completed in 1492 for the altarpiece at Ghent cathedral.

The first view is it closed, the second is it opened. The cover frame which may have contained clockwork opening mechanisms and music elements was destroyed during the Reformation.

I think those two panels represent heaven and hell, Kelp. I'm pretty sure the left is the pearly gates, not a church.

I suppose that the right hand side is a burning church?
